Job Description:

Kaiser Permanente Georgia is seeking an Inpatient Case Manager to join our dedicated team at our Southwood facility on an on-call basis. This position is ideal for a dedicated healthcare professional who excels in coordinating complex patient care and discharge planning to optimize the management of all medical needs and ensure compliance with the healthcare provider’s recommendations. As a part of our team, you will contribute directly to providing seamless and comprehensive care in alignment with Kaiser Permanente's commitment to improving health outcomes. Conduct patient assessments to determine appropriate levels of care and specific patient needs. Create and implement personalized care plans in consultation with healthcare providers. Collaborate with a multidisciplinary team to facilitate comprehensive patient care. Monitor patient progress and adjust care plans as necessary. Ensure the effective transition of patients from hospital to home or other care facilities. Educate patients and families about conditions and treatment options. Maintain thorough documentation of patient care and interventions. Strong interpersonal and communication skills Expertise in care coordination and patient advocacy Ability to work effectively within a multidisciplinary team Proficient in electronic medical record (EMR) software Knowledge of healthcare regulations and accreditation standards Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Current Registered Nurse (RN) license in the state of Georgia Certification in Case Management (CCM) preferred but not required
